Non-insurance Rates and Packages
For those of you who elect not to use insurance or do not have insurance, we offer individual rates and packages:
INDIVIDUAL PACKAGES*
*For all packages, there is no expiration date and a credit will be applied to your account. All payments due at time of purchase.
INSURANCES ACCEPTED
We currently accept all major PPO insurances, including:
- Anthem Blue Cross
- Blue Cross Blue Shield
- Cigna/ASH (American Specialty Health)
- United Healthcare/Optum
- Aetna
We also accept the following HMO plans:

Remaining insurance providers accepted include:
- Medicare
- Medi-Cal
- Tricare
- Worker’s Compensation
- OneCall
- Medrisk
- Onsite Physio
- Corvel
- Motor Vehicle Accident (i.e. car insurance providers)
Non-insurance:
- Personal Injury/Lien Cases
